1. Key People to Identify
- Ultimate Beneficial Owners (UBO): Any person who owns more than 25% of the company or exercises effective control.
- Shareholders: Any person who holds 5% or more of the company's shares.
- Authorized account operators: People with authority to manage the company's accounts.
- Elected representatives or administrators: Individuals who officially represent or manage the company.
2. Required Information for Each Person
- Full name
- Identification number (with a copy of the document)
- Tax identification number
- Place of birth
- Date of birth
- Nationality
- Country of residence
- Role or percentage in the company
For complex ownership structures: You must show the complete ownership chain that leads to the individuals who control the company.
If there are none: If a category does not apply, simply indicate "None."
For Self-Employed Workers (Trabalhador Independente) and Sole Proprietors (ENIs)

- Authorized persons: Any person who has authority to operate your account or control your business on your behalf.
2. Required Information
- Full name
- Identification number
- Date of birth

- Country of residence
